All Students Agree
Motion Dance Academy is not responsible for children left before or after class.
Motion Dance Academy is not responsible for personal property.
In attending Motion Dance Academy, taking dance or exercise classes and otherwise using the facilities, equipment therein, or participating with outside the studio, I do so at my own risk.
Motion Dance Academy will not be liable for any damages arising from personal injuries incurred by me in, on or about the premises of the Motion Dance Academy dance studio/program, including performances outside the studio.
I assume full responsibility for any injuries or damages which may occur to me in, on or about Motion Dance Academy studio/program, and I do hereby fully and forever release and discharge Motion Dance Academy, its directors, officers, dance instructors, employees and agents from any and all claims, demands, damages, rights of action or causes of action, present or future, whether the same be known, anticipated or unanticipated, resulting from or arising out of my use of said Motion Dance Academy dance studio/program, dance classes, performances, facilities and equipment thereof.
Motion Dance Academy photographs and videotapes its students in class and performance, and uses these photographs and videotapes in its advertising and promotion. By signing this contract and enrolling your child at Motion Dance Academy, you agree that your child’s picture/video may be taken and that you consent to the use of these pictures and videos by the Academy for promotional purposes.

Attendance
Students are expected to attend all of their classes. Good attendance is imperative, as absences and tardiness can hold back the student themselves and an entire class. The studio cannot jeopardize its responsibilities to the rest of the class for a single student. Parents must notify instructor(s) by phone or email if their child will be late or absent from class.

Classroom Etiquette
Students must be on time and prepared for class. If a student is late for class, they must come in quietly and politely ask the teacher if they may join the class.
Students must be dressed in the required attire (see Dress Code) and have their hair securely pulled away from face.
No street shoes, jewelry, gum or food (unless permitted by instructor).
Students must use the restroom before class begins. Leaving not only disrupts class but it may cause the the student to miss out on valuable warmups or combinations.
Students must maintain a respectful attitude toward instructors at all times.
Touching mirrors, walls, or equipment is not permitted.

Waiting Room
The viewing window gives parents an opportunity watch their child's class. However, if their presence distracts students then they should step away from the window and avoid students seeing them. Touching the viewing window is not permitted.
All family and friends must remain quiet in the waiting room.
Food or drinks are permitted in the waiting room but must be cleaned up afterward.

Drop Off & Pickup
Students must be dropped off at least 5 minutes (no more than 10) before class begins and must be picked up no longer than 10 minutes after class ends. Children must be be supervised when not in class.

Communication
Parents must read their email clearly. Email, Facebook and the MDA Blog will be parents main source of receiving important information. Parents must notify Motion Dance Academy with any contact changes. It is the responsibility of the parent (or adult student) to be aware of studio activities. Reminders & newsletters will be emailed, posted in the studio waiting room and on the Motion Dance Academy website and blog.
Payment Mothods
Parents may pay all fees online with auto-pay or with check or cash put in the studio dropbox
Tuition
Tuition is due on the 1st of each month. A $10 late fee will be charged after the 5th. Tuition will remain the same whether it is a long (5-week) or short (3-week) month and regardless of student absences. Parents may schedule a make-up class within 30 days of a missed class. There is no extra charge for five week months and no refunds for scheduled holiday breaks. No charge for additional rehearsals for upcoming performances.
Registration
Annual Registration Fee: $20 for single student ($10 for additional child), non-refundable (DUE AT TIME OF REGISTRATION)
Additional Fees
We will have a Christmas Recital in January and a Spring Recital in June.
Costumes may vary in cost but each is approximately $35-$55
Costume fees are collected the first week in October. We will not order a costume without the fee and filled out form.

Spring Recital Fee(s) are $20.00 per family is due in March
These fees help cover the cost of auditorium rental, lighting, props, stage crew, programs, etc. The fees also includes UNLIMITED admission for your family and friends.
